Uncle Jack leaves behind his sister Margaret (Maggie) Chavez and sister-in-law Mary Chavez. He also leaves behind numerous Nieces ad Nephews, and he will be greatly missed by all. We all have wonderful memories to share of our uncle.
Uncle Jack is preceded in death in by his Father Antonio Chavez, Sr. and his Mother Flora Chavez (Chavez). His Sisters Mary Frances Chavez, Eloisa Chavez and Mary Bachicha. His brothers Ramon Chavez, Antonio Chavez, Jr., Abel Chavez, Juan C. Chavez, Enriques Chavez, Frank C. Chavez, Cayetano Chavez & Todosio (Toby) Chavez.
Uncle Jack was a lifetime resident of Moriarty, New Mexico. He was also a member of Our Lady of Maunt Carmel Catholic Church.
He was a very reserved man and enjoyed the company of others. However, he especially looked forward to visiting with his good friends Tom Colletti, Rudy Ortiz & Vicente Tapia. A special thanks to ALL his neighbors for a life time of kindness.
Uncle Jack loved cleaning his house. If he knew his Nurse Andrea, a Social Worker or someone else was coming over he’d make sure everything was tidy.
He also loved working in his yard, but he was limited to what he could do due to his health.
Uncle Jack was a man of many talents and was a very handy man. He especially enjoyed taking care of his wild cats. The more he ‘d feed them they would never leave.
Uncle Jack worked various odd jobs in his life time. He also worked for the Colletti family for several years. He was Blessed to have met such an AMAZING FAMILY. The Colletti’s took him in and treated him as if he was their family. But most importantly they treated him with utmost respect, dignity and kindness.
A Special thanks to Gabriella & Danny Apodaca for visiting him with your kindness and being there when he needed assistance. He was extremely grateful & appreciated that you both made time to be there in his time of need.

A special thanks to Gentiva Hospice especially to Andrea Gousha his Nurse. She went way beyond to make sure Uncle Jack had taken his pills correctly & made sure if he had eaten. If he hadn’t eaten, she would fix him something to eat and kept him company while he ate. She became his good friend. I also want to thank April Castro his Nurse Aid. She was a blessing to my Uncle Jack.
Uncle Jack was fortunate that at his last hours Gentiva Hospice made sure he was kept comfortable and that he went home extremely peaceful.
